Definition
Root Cause Analysis (RCA) in Risk Management is a Risk Analysis method used for identifying the underlying (root) causes of risk events. The methodology is used across many domains.
RCA Steps
- Identify and describe negative outcomes
- Establish a sequence leading from a prior state to the negative outcome
- Distinguish between root causes and other causal factors
- Establish a causal graph between the root cause and the problem
