A computer virus was discovered on an employees work computer account on December 14, 2012. One of the files on the employees computer contained patient names, addresses, telephone numbers, dates of birth, medical record numbers, names of health insurers, diagnoses, and other clinical information. A limited number of Social Security numbers were also exposed. (English)
