The Credentialing Coordinator was working with two provider application files around 5:00 in the evening. She left her office to go to the COSs office which left one other employee in the office. When she returned one of the provider files was missing. The other employee did not know of anyone coming into the office. The file contained III to include Name, DOB, SSN, Address, Telephone #, reference letters, previous employers etc. (English)
